Please complete and submit a copy of this form for each delegate attending the ASA Conference 2012. Alternatively, you can call our event managers, Corporacare, and make your booking by phone (details below) or download and print a copy of the form, complete and send it to Corporacare at the address listed.

Conference rates
Payment

The ASA does not presently have a secure transaction facility for credit or debit card payments via the web. If you wish to pay by card, please contact Corporacare [Tel. +44 (0)117 909 9550]. Please do not send card details by email.

If you wish to pay by cheque, please post cheque/s with completed booking form/s to the address below. Cheques must be in Pounds Sterling and drawn on a UK bank. If paying in any other currency please add £15.00 to cover bank charges. Cheques should be sent to: ASA Conference 2011 c/o Corporacare, 47 High St, Westbury on Trym, Bristol, BS9 3ED, UK.
Hotel Accommodation

Hotel accommodation, which is not included in the delegate fee, will be available at a special negotiated rate (£140 for a single and £150 for a double, including breakfast and VAT) at the nearby Holiday Inn at 57 Welbeck St, London (5-minutes walk from the conference and close to Oxford Circus). Delegates are responsible for all of their hotel costs and should pay the hotel directly during their stay. However, to take advantage of the special conference rate, delegates should reserve their room through Corporacare by completing their details, below. No money will be charged at this stage.

Cancellations Policy

The following applies to both conference registration and accommodation charges, where accommodation has been booked through Corporacare: Cancellations made more than 6 weeks prior to the first day of the conference will be given a 75% refund. Cancellations made 2-6 weeks prior to first day of conference will be given a 50% refund. Cancellations made less than 2 weeks prior to first day of conference will not be given a refund. Substitutions may be made at no charge up until one week before the conference (please contact Corporacare).
